Terms and conditions

By booking an appointment with Theodora de Jasay MVetPhys, of de Jasay Veterinary Physiotherapy, you agree to the following Terms and Conditions:

1. Veterinary Collaboration

In order to maintain compliance with the Veterinary Surgeons Act 1966, Veterinary Surgeons (Exemptions) Order 2015, and the Register of Animal Musculoskeletal Practitioners' (RAMP) code of conduct and standards of practice, if your animal has any diagnosed condition or is currently under veterinary care for any condition, I will require a consent form to be completed by their primary veterinarian before beginning physiotherapy. Owners must inform me prior to the session if their horse or dog has any such conditions.

As a veterinary physiotherapist, I work alongside your veterinarian to provide the best possible care for your horse or dog. If, during my assessment, I identify a new or undiagnosed area of concern that I believe requires veterinary attention, I will request that you consult with your veterinarian before proceeding with physiotherapy.

I often like to send your veterinarian a summary report following my first session with your animal, and every few sessions after to update them on progress. This means we can work collaboratively to get the best outcomes. These summary reports detail key clinical findings, treatments carried out and our physiotherapeutic approach going forward. If you would prefer I didn’t share a summary report please inform me. If this is the case, and in order to uphold my professional obligations, I will instead contact your vet to simply let them know the appointment was carried out, that clinical notes were documented but you would prefer that my report is not shared at this time, and that I will advise you to arrange a veterinary assessment should any new clinical signs or adverse changes develop.

2. Travel costs

I try to group my appointments as best as possible by geographical area, this allows me to absorb the cost of travel. If you require an appointment outside times when I plan to be in your area, I may need to charge additional fees. This is particularly the case for appointments outside the M25, that require driving in the congestion zone, or when travel times exceed 45 minutes. Congestion charges will be added in full. If travel times exceed 45 minutes, a £10 surcharge will be added.

A guide to fees by distance outside the M25 is below:

  • 0-6 miles = free

  • 6-14 miles = £5

  • 14-20 miles = £10

  • 20+ miles = on request

3. Cancellation Policy

If you need to cancel or reschedule your appointment, the following charges will apply:

  • 72 hours before the appointment: 20% of the session fee will be charged.

  • 48 hours before the appointment: 50% of the session fee will be charged.

  • 24 hours before the appointment: 100% of the session fee will be charged.

  • No-show: 100% of the session fee will be charged.

4. Block Booking Policy

  • A stand-alone initial assessment session is required before block bookings of follow-up sessions can be made.

  • Block booking for a discounted rate of 5% for 5 follow-up sessions, and 10% for 10 follow-up sessions is available.

  • Session dates do not need to be decided upon up-front, however all block-booked sessions must be used in the period of 12-months from purchase.

  • Block bookings are non-refundable.

5. Fees and Payment Terms

I reserve the right to periodically review and alter my fees to reflect inflation and the value of my services.

Payment options include:

  • Cash / bank transfer on the day of the appointment.

  • Invoice sent ahead of the session and due on the day of your appointment.

Failure to complete payment within 14 days may result in additional charges:

  • A 20% late fee will be added to overdue invoices on the 14th day, and every 7 days after that until payment is received.

Please note: If you have booked a session at either WagWorks Club, payment will be made to the reception team.

6. Use of Images and Videos

During physiotherapy sessions, I may take images or videos of your horse/dog for use on my website, social media accounts and in educational resources.

If you prefer that images or videos of your animal are not used, please let me know when completing my New Client Form, and I will fully respect your wishes.

7. Liability & Owner Responsibility

Owners must ensure their horse or dog is safely restrained and handled during appointments. Ensure that I have been made aware of any behavioural considerations for your animal – examples may include nervousness, sensitivity to handling of particular body regions, or aggression: this allows me to take the necessary precautions to maintain safety, as well as your horse or dog’s comfort.

  • While I take every precaution to ensure a safe and professional service, as the owner you accept full responsibility for your animal's behaviour during the session.

8. Data Protection & Confidentiality

Any information shared during consultations is treated as confidential and will not be shared with third parties without your consent, except with your veterinarian, if required, for the care of your horse/dog.

Your personal details will not be shared without your explicit consent.

9. Privacy Policy

de Jasay Veterinary Physiotherapy is committed to protecting the privacy of clients and users across all aspects of the business.

Please view the privacy policy on our website which explains what data is collected, how it is used, stored and shared, and the rights of individuals under UK data protection law.

10. Agreement to Terms

By booking an appointment you confirm that you have read, understood, and agreed to these Terms and Conditions. I will ask you to confirm this when you complete my New Client Form, if you have not done so already. For any questions, please contact me before your appointment.
